- Proof of completion for any state-mandated topics required by Ohio.
- A personal CE log or spreadsheet tracking course dates, titles, providers, hours, and topic mapping.
- If you used a competency or practice-hours pathway in lieu of contact hours, documentation supporting that pathway (employer verification, practice logs, certification copies).
Ohio-specific mandated topics
Beyond a total hour count, Ohio requires specific subject-matter CE within the cycle. Make sure each mandated topic is covered by a certificate that names the topic explicitly.

If your hours fall short
First-time shortfalls are usually handled administratively. Boards generally allow a defined window to complete makeup hours and submit updated documentation. Honest, prompt communication is far better than silence.
If the shortfall is significant or repeated, the board may move toward formal action. At that point, calm documentation of how you are coming into compliance, plus a written plan to prevent recurrence, makes a meaningful difference.

- If the Board finds that a licensee has not complied with the CE requirements, it may impose a fine, reprimand the licensee, or place the license on probation, suspension, or revocation, as per OAC 4723-14-07. The Board will consider mitigating factors when determining the sanction.
- The licensee is given an opportunity to show compliance before a final action is taken. The board may also require the licensee to make up any deficient contact hours.
- Disciplinary action taken by the Board is public information.
- Falsification of CE is a violation of the Nurse Practice Act and will result in disciplinary action.
